History

  • 57 year old obese lady
  • Increasing dyspnea, facial swelling and flushing

Findings


Diagnosis

Lipomatous hypertrophy of the interatrial septum
  • Benign cardiac mass characterized by fatty deposits in the interatrial septum with a thickness of more than 2cm Incidence: 1-8%
  • More common in elderly, female, obese
  • Usually asymptomatic-rarely arrhythmia or obstructive symptoms
  • Differential: Myxomas (tend to arise from the foramen ovale, exophytic) Lipomas- encapsulated, rhabdomymas and fibromas-predom infants and children and in the ventricles, metastatic disease

Treatemnt

  • Treatment Almost always nothing- benign incidental finding. This case surgical: SVC/ Rt. atrial obstruction or severe rhythm disorders
