  • 42yo male
  • C/O: dyspnoea
  • PMHx: previous MI
  • ECG: NSR, oST ↑
  • Coronary angio: RCA 90%, Circ 80%
  • Which is culprit lesion?
  • Which should be revascularized? Both??



Findings

  • Dx: Matched perfusion defect in Circ territory, with oedema, transmural scarring and no-reflow zone→ acute non-viable infarct (Circ)
  • Also scar in inferior wall (RCA)

Management

  • no revascularization
