History
- 42yo male
- C/O: dyspnoea
- PMHx: previous MI
- ECG: NSR, oST ↑
- Coronary angio: RCA 90%, Circ 80%
- Which is culprit lesion?
- Which should be revascularized? Both??



Findings
- Dx: Matched perfusion defect in Circ territory, with oedema, transmural scarring and no-reflow zone→ acute non-viable infarct (Circ)
- Also scar in inferior wall (RCA)
Management
- no revascularization
